Strategy

Every action plan requires a strategy….

Our strategy is to walk along side you on your journey to Brighter Days

  • A substance use comprehensive assessment is a deep and thorough evaluation designed to understand the full scope of an individual's relationship with substances. It goes beyond surface-level screening, examining medical, psychological, behavioral, and social factors that contribute to substance use.

Think of it like mapping out a person's journey—their history of use, triggers, coping mechanisms, and the impact on their personal and professional life. It often includes structured interviews, self-reported questionnaires, and clinical observations to paint a holistic picture. The goal is to identify risks, strengths, and needs, guiding personalized treatment plans that foster recovery and support well-being.

  • Substance Use Individualized Treatment Plan (ITP) is a personalized, structured document developed collaboratively between a person receiving treatment for substance use and their care team. It outlines specific goals, strategies, and interventions tailored to the individual's unique needs, preferences, and circumstances.

Key Features of an Individualized Treatment Plan for Substance Use:

    1. Personalized Assessment:

      • Begins with a comprehensive evaluation of the individual's substance use history, mental and physical health, social environment, and personal goals.

    2. Goal-Oriented:

      • Includes short-term and long-term goals that the individual wants to achieve, such as abstinence, improved relationships, or stable employment.

    3. Collaborative Development:

      • Created with input from the individual, clinicians, and often family members or support systems to ensure it reflects the person’s values and priorities.

    4. Specific Interventions:

      • These could be various counseling modalities, community referrals and treatment coordination.

    5. Measurable Objectives:

      • Breaks down goals into actionable, time-bound steps that can be tracked and adjusted as needed.

    6. Regular Review and Updates:

      • The plan is dynamic and should be reviewed periodically to reflect progress, setbacks, or changes in the individual’s situation .

    7. Holistic Approach:

      • Addresses not just substance use, but also co-occurring mental health issues, housing, employment, legal concerns, and family dynamics.

  • Mental health services in substance use treatment refer to the integrated support and therapeutic interventions provided to individuals with co-occurring mental health and substance use disorders. These services aim to address emotional, psychological, and behavioral issues that contribute to or result from substance use, using evidence-based approaches such as counseling, psychiatric care, and behavioral therapies to promote holistic recovery and long-term wellness.

  • Peer Recovery Support Services (PRSS) in substance use treatment are non-clinical services provided by individuals who have lived experience with substance use and recovery. These peers offer mentorship, emotional support, practical assistance, and guidance to others currently navigating recovery. Through shared understanding and mutual respect, peer support workers help individuals stay engaged in treatment, build recovery skills, and reduce the risk of relapse.
